Public Records & Reports Request
The Ohio State Highway Patrol maintains:
- Complete crash reports and crash photographs from crashes investigated by the Ohio State Highway Patrol only. Crash reports and photographs are retained for five years.
- Criminal investigation reports taken by troopers and Ohio Investigative Unit agents.
Crash Reports
Please wait a minimum of seven business days from the date of the crash before attempting to order a crash report and/or photographs to ensure the availability of the completed initial report. Also, please be aware some crashes require additional investigation beyond the initial report. Additional information resulting from these investigations may not be available until such investigations are complete.
To order crash reports and/or photos online you must have:
- a web browser capable of running Javascript,
- a valid credit card, and
- a viewer that is able to view PDF files.
School Vehicle Traffic Crash Reports
A School Vehicle Traffic Crash Report Request OHP 0413 must be completed by a parent or guardian requesting student traffic crash records kept by the Ohio Department of Public Safety (ODPS) and the Ohio State Highway Patrol (OSHP).
This form must be completed by parents and guardians of an individual:
- Who is less than eighteen years of age, and
- Who was in a traffic crash that involved a school vehicle, e.g., was an occupant on a school bus.

NOTE: You must complete the purchase process to receive a copy of the crash report. The requestor will receive a redacted copy of the crash report after the purchase has been processed. Once the OHP 0413 School Vehicle Traffic Crash Report Request is received and verified, another copy of the crash report with the information from the requestor’s child ONLY will be sent to the requestor at no additional cost.
Public Records
The Central Records Unit is responsible for maintaining records for the Ohio State Highway Patrol and the Ohio Investigative Unit. Victims/Victim’s Representatives Request for Records
Requesting Unredacted Copies of Records:
An OHP 0413 Request for Records must be completed by a victim or victim’s representative requesting specific unredacted records kept by the Ohio Department of Public Safety (ODPS) and the Ohio State Highway Patrol (OSHP).
This form must be completed by the victim, victim’s representative, or victim’s attorney. This form may be used to request the following:
- Unredacted copies of the victim’s rights request form (R.C. 2930.04(C)(2).
- Unredacted Case Documents (R.C. 2930.07(E)(3)).

Requesting Additional Redactions to OSHP Records:
An OHP 0415 Request for Redactions must be completed by a victim or victim’s representative requesting additional redactions to specific records kept by the Ohio Department of Public Safety (ODPS) and the Ohio State Highway Patrol (OSHP) pursuant to R.C. 2930.07(D)(1).
This form must be completed by the victim or victim’s representative. You must specify the specific incidents, documents, or records that you are requesting be redacted.
